Does the form have to be notarized?

Yes. All NFCYM/NCYC Liability Waiver and Permission forms must be notarized in order to complete registration. No attendee will be allowed to participate in the National Catholic Youth Conference (NCYC) without a signed and notarized form on file.

What “legal rights and remedies” might I be giving up by signing this waiver and permission form?

This is a question you must take up with your legal counsel, but, in general, signing this form indicates that you understand the nature of the NCYC and the risks involved in participation and that you knowingly, freely, and willingly assume all of the risks that may be involved in attending the NCYC.

Under “Waiver of Liability/Hold Harmless,” what does “for value received” mean?

“For value received” is standard contract language. It means that all parties to the contract receive a benefit by entering into the contract, such as access to NCYC programming. There is a value to that programming that may not be captured in dollars.

There are some things in this form with which I don’t agree. May I strike them out?

No. This liability and waiver form may not be altered. With 20,000 or more people expected to participate in NCYC, it is not feasible for each form to be modified to the individual participant’s wishes. If you are unable to sign the form as is, you or your child may not participate in NCYC.

Under “Medical Permissions (Limited),” it says NFCYM et al. is not responsible for notifying emergency contacts. If my child is ill I want to be sure I am called.

Because NFCYM et al. never chaperones young people directly, we are rarely informed of a participant’s illness. Therefore, in the event that a participant becomes ill, it is the responsibility of diocesan group leaders to notify the participant’s emergency contact.

What is the “insurance” referred to on the liability and waiver form?

NFCYM suggests that participants consider purchasing insurance that specifically covers the risk of traveling to/from and participating in NCYC. Depending upon the product selected, here are just a few of the benefits travelers may enjoy with travel protection from Access America:
• Reimbursement of the non-refunded portion of your travel investment if your trip is cancelled or interrupted for a covered reason
• 24-hour access to emergency medical referrals and assistance
• Reimbursement for unexpected travel expenses, including lost luggage, rental cars, and medical emergencies
• Referrals to legal services

Who or what is Access America?

To the best of our understanding, Access America is a travel insurance company offering travel protection products through more than 22,000 travel agencies throughout the United States. Travel protection is a combination of travel insurance and 24-hour emergency assistance services designed to give travelers peace of mind when they travel, and helps travelers manage unexpected risks. Access America offers a variety of products to meet all types of travel needs. Not only are the belongings travelers take with them covered, but they are also protected in case of a medical emergency and other situations. Exclusions may apply, depending upon the policy purchased. These services are not typically fully covered by credit card companies, homeowner's policy, and/or medical insurance.
We further understand that Access America is a division of World Access Service Corp, a member of the Mondial Assistance Group. Part of the Allianz Insurance Group, the Mondial Assistance Group is a large travel insurance and assistance company, providing travel protection products and services to organizations in the travel, health care, and financial service industries. The insurance coverage that World Access provides is underwritten by BCS Insurance Company, which is rated A- "Excellent" by A.M. Best and Company.

What is NFCYM’s relationship with Access America?

NFCYM has no affiliation with Access America. NFCYM merely suggest it as a means of your managing any risks that you assume by traveling to and attending the NCYC.
